Who can be a Patient?

Can you say “YES” to all of these statements?

  1. I am at least 19 years old.
  2. do not have health insurance, and/or are not eligible for insurance. This means I do NOT have, or cannot get:
    • Oregon Health Plan (OHP/Medicaid), or Medicare;
    • Any of the Affordable Care Act (Obamacare) plans;
    • Insurance through my work, my spouse’s work, or any other organization (including VA).

Can you say “YES” to all of these statements?

  1. I am at least 19 years old.
  2. do not have health insurance, and/or are not eligible for insurance. This means I do NOT have, or cannot get:
    • Oregon Health Plan (OHP/Medicaid), or Medicare;
    • Any of the Affordable Care Act (Obamacare) plans;
    • Insurance through my work, my spouse’s work, or any other organization (including VA).
  3.  You have lived or worked in Deschutes, Jefferson, or Crook County for at least 3 months.
  4. You agree to complete a VIM eligibility assessment each year.
  5. Your total gross household income (before taxes) is less than 300% of the federal poverty level.
    • Calculate your gross income for your household.  This means add up all of the money that people in your household make before any taxes are taken out.
    • If your household income is less than the amounts listed on the chart, you may be eligible for OHP.

Our clinic and charitable pharmacy provide care and medications for uninsured working adults who are not eligible for Medicaid (locally known as "Oregon Health Plan") and cannot afford other health insurance.

  • you are 19 years old or older,
  • you have no health insurance, and
  • you live and work in Central Oregon
    • residing in Deschutes, Crook, or Jefferson Counties
  • valid photo ID
  • proof of income (last 3 months)
  • proof of address
  • most recent tax return and/or W-2
  • a bank statement for the most recent month

Patient eligibility depends on various aspects like your income, household size, Medicaid eligibility status, and other factors. We recommend calling us (541-330-9001) to discuss your unique situation and see if you qualify as a VIM Cascades patient.
