Patient Eligibility

VIM serves low-income, un-insured residents of Deschutes County. Besides meeting our financial and household size guidelines, you must also meet the requirements stated below:

  • Show proof of residency or of work in Deschutes County for at least 90 days prior to applying for eligibility.
  • Have no health insurance including private or employer-sponsored health insurance, Oregon Health Plan (OHP), Medicare, Medicaid, OMAP, DMAP or DMIP, and COIHS. If you are a Veteran, you must show proof of being denied VA benefits before being considered for VIM services.
  • If you are a patient at any other safety net clinic such as Mosaic Medical, La Pine Clinic, or City Care Clinic of Redmond, you already have access to primary care and are not eligible for services at VIM.
  • We do not take children who are qualified for Oregon Health Plan (OHP).

If you think you meet these requirements, please follow the instructions below:

  • Call (541) 330-9001, and press 5. Leave your name and phone number. Do not leave multiple messages as we will call you back as soon as possible. We get a high volume of calls so we ask for your patience. You may call at anytime.

Once we schedule you for an eligibility screening appointment, you will need to bring:

  • Proof of Deschutes County residency, or proof of work in Deschutes County for 90 days (driver's license, a rental lease, or utility bill, payroll check stubs).
  • Pay stubs and proof of all sources of income for ALL contributing adults in your household. Most current income tax return.
  • Last 3 months of bank statements (debit, checking, savings). Include investment accounts, annuities, and trust statements, if applicable.
  • Printout of last 3 months of unemployment income, if applicable.
  • Proof of income from SSI, school loans, child support, and/or additional payments.
  • Documentation of ALL cash income: ledger book (source; date of services; amount of income for each payment)

If you are self-employed, you will also need to bring:

  • 3 months of personal AND BUSINESS bank statements (debits, checking, savings)
  • Profit/Loss statements related to the business for the most current 3 months.

Expectation of Patients

Being on time for appointments is very important. We ask that patients arrive early. If a patient arrives late we may not be able to see them.

Referrals to doctors and other care providers outside of VIM must be coordinated through VIM. Unauthorized referrals are subject to billing at the provider’s discretion.

Our doctors are volunteers. This means that our schedule can change with little or no notice. We may need to reschedule appointment times or change the medical provider that a patient is scheduled with.

If a patient makes an appointment, they are required to call at least 24 hours before the scheduled time so that we can give the appointment to another patient. Not showing up for appointments may cause patients to lose eligibility at VIM.

The volunteers and staff at VIM are here to help patients get the information, care and support to get better. We ask all patients to take an active role in improving their health and wellness.

We expect all VIM patients to work with our volunteer providers by following their treatment plans. Refusing to follow treatment can result in a patient losing eligibility at VIM.

Patients should ask questions. This provides a better understanding of what is going on, as well as, helping our doctors know more about patients and their health.

Patients are asked to make donations to support the operation of the clinic. All of the money needed to run the VIM Clinic comes from donations. Even a small donation can go a long way.

Charitable Pharmacy Information

VIM patients can receive assistance with getting prescription medications through our charitable pharmacy. Our volunteers will help patients fill out forms that can qualify patients for medications and we keep a small supply of generic medications for dispensing to patients. We also send many prescriptions out to local pharmacies. We do not prescribe or dispense any narcotics, pain medication or medical marijuana at VIM. Refills must be called in two working days ahead.
